Over the past three years, four coalitions have been active in the community addressing the issues identified in the 2015 Logan County Community Health Risk and Needs Assessment. The work of the coalitions has been tracked and can be seen in the final Report of Accomplishments of the 2015 Community Health Improvement Plan. The report is available for viewing on the Logan County Health District website – www.loganhealth.org, www.maryrutan.org, www.mhdas.org, www.uwlogan.org or www.chwplc.org.

Logan County completed a new assessment in 2018 to better understand current challenges facing the residents of Logan County. There are five agencies providing Project Management for the Needs Assessment: Community Health & Wellness Partners of Logan County; Logan County Health District; Mental Health Drug and Alcohol Services Board of Logan and Champaign Counties; Mary Rutan Hospital and United Way of Logan County. The FINAL 2018 Community Health Risk and Needs Assessment is now published and available at the websites listed above.

In October a new Community Health Improvement Plan (CHIP) will be completed. The CHIP provides strategies and actions for the next three-year period that are specific and designed to address the areas of risk and need identified in the 2018 Needs Assessment. The priority areas identified by the community are:

1. Mental Health

2. Substance Abuse

3. Obesity and Chronic Disease

4. Access, Resources and Awareness

5. Maltreatment of Children

6. Housing and Homelessness

7. Workforce Readiness

By completing this process major risks and needs of a community are identified and documented. Priorities were agreed upon by community participants in a Call to Action. For each of the identified priorities, specific plans are developed that are designed to address the areas of risk and need in order to improve the community’s health, safety and well-being. Seven coalitions or workgroups were formed to address these priorities:

Coalition for Opiate Relief Efforts (C.O.R.E.)

Healthy Living Coalition

Access & Resource Coalition (ARC)

Safe & Healthy Kids

Workforce Development Committee

Continuum of Care Coalition
